Hiring Paperwork and Procedures for US Citizens or Permanent Residents

In order to set up your Fellowship support, Graduate Student Researcher Appointments (GSRs) or Teaching Assistant Appointments (TAs), there is some paperwork that must be completed:

Please leave the 'Employee ID number' fields blank - they will be filled in after a payroll ID number has been generated. (This is not the same as your student ID number)

Please leave all date fields blank, you will be filling them in during your hiring appointment.

NO STAPLES - They will have to be removed.

1) Patent Agreement:

All students, faculty and staff at UCSD must complete a patent agreement prior to receiving salary or stipend.

Complete your name, date prepared, and department at the top. Read but do not yet sign the Oath of Allegiance section - you will be signing it at your hiring appointment. At the very bottom, print and sign your name, and date it. http://www- bfs.ucsd.edu/pay/forms/pdf/oath.pdf

2) Direct Deposit/Payroll Wage Disposition 

If you would like electronic deposit of your checks. Follow the directions in A and then complete the form in B, since your checks will most likely come from two separate money disbursement systems on campus.

3) I-9 Employment Eligibility Verification

AFTER you have completed the forms, please send Kim Graves your availability for a few weekdays between the hours of 9am and 3pm, to come to her office with the completed forms. In addition please bring the following IDs/documents:

  • ONLY valid passport

    OR

  • BOTH Social Security Card and a valid drivers license

 Save yourself a trip, make sure you bring the correct ID. If in doubt, ask.
